• Complete name of the notary public;
• Roll of attorneys number, which you may verify at https://sc.judiciary.gov.ph/lawyers-list-2/;
• Notarial commission number;
• Place of notarial commission;
• Expiration date of the notarial commission;
• IBP membership number (either lifetime or for the current year);
• Professional tax receipt number for the current year;
• MCLE compliance number;
• Office address;
• BIR form
